Narrative

Employee was pulling on fuel nozzle on fuel truck and strained his shoulder. Employee refused medical treatment at time of actual accident, claiming it was nothing. He later reported pain (on June 17, 2004) claiming it was from the original April accident. The investigation of this claim did not begin until 6/17/04.
