Employee was lifting a section of 60"" belt structure and strained his shoulder. This employee did not seek medical treatment for several weeks. Initial treatment was physical therpy which did not help. Employee required surgery and this was done several months later. Employee's fisrt day of restricted duty for this injury was June 14th, 2004
