Injured employee was chipping shotcrete and fiber out of a hardline. When the tool broke through the clog, employee's right hand followed through to entrance of pipe. Contact with shotcrete, fiber, and pipe caused employee's right little fingernail to dislodge. Complete removal of fingernail was necessary with two sutures in nail bed.
