## Narrative

On July 05, 2004 the employee reported he was having difficulty with his shoulders.  He attributes the pain to years of repetitive motion using hammers and impact wrenches in his position as a welder/mechanic.  He reports the pain has become constant, and he has been seen by a Physical Therapist? To date neither time nor wages have been lost as a result of incident.
