ON THIS DATE THE EMPLOYEE REPORTED THAT IT WAS DETERMINED THAT HE HAD CARPAL TUNNEL DISORDERS IN BOTH WRISTS. HE STARTED MISSING WORK ON 7-19-04 TO HAVE SURGERY ON HIS WRIST.
