## Narrative

The injured was removing a small axle and gear case from an underground three wheeler with the assistance of an overhead crane.  He had the gear case wired in a position in an attempt to prevent it from rolling over as he removed it from the machine.  As he was setting it down on the floor, the wire came loose letting the case roll over striking his foot.
