Narrative

A roof fall occurred in entry #1 of the 2nd N.E. mains at survey station 12+60. The fall measured 140' long x 19' wide x 9"" thick. This area had been bolted with 6' fully grouted and 9' double lock bolts. All approaches will be timbered.
