WHILE CROSSING WORK DECK EE CAUGHT HIS TOE ON A BEAM AND FELL ON HIS LEFT KNEE AND RIGHT SHOULDER.  EE WAS WORKING AT #7 MINE AT THE TIME OF HIS ACCIDENT.  HE NOW IS WORKING AT #5 MINE.  HE WORKED UP TO 8/18/04 BEFORE HE STARTED LOSING TIME ON HIS INJURY.
