A roof fall occured in the A-2 East Main entries at cross-cut 10 in the intersection of the #2 entry measuring 19'x19'x6'. This area was mined in May 2000 and supported with 6' tension rebar bolts and lincoln logs. The fall was caused by an undetected slip.
