A roof fall occured in the East Mains in #8 entry in 2/3 of the intersection of cross-cut #27 measuring 13ft. x 19ft. x 6ft. This area was mined in April 2004 and supported with 6ft. tension rebar bolts and 6in. x 6in. square timbers. This fall was caused by an undetected slip.
