EMPLOYEE WAS GREASING WATER TRUCK WITH GREASE GUN, GREASE FELL INTO HIS LEFT EYE. EE DID NOT SEEK MEDICAL ATTN. UNTIL 9/7/04, AT THE EYE CENTER, WHERE FOREIGN MATERIAL WAS REMOVED FROM HIS LEFT EYE.
