HE WAS TRYING TO DISLODGE DEBRIS IN A PINNER STEEL, HE WAS USING A WRENCH TO FREE UP STEEL, WHEN HE HIT THE STEEL THE INSERT OF THE WRENCH CAME OUT AND STRUCK HIM.
