## Narrative

Operator was bent over sorting steel balls on the floor with other employees. Contractor working inside mill welding a plate dropped the plate; it fell through door in mill. Plate glanced off steel bolts striking employee in lower back. Plate fell 20 ft; it was 18"" by 6"" and 1/4"" thick.
