A roof fall occured in the 6 North Panel in the #9 entry starting at the intersection of crosscut #72 and proceeding approximately 1/4 of the way through the intersection of crosscut #74. The fall measured approximately 115' X 19' X 5'. The area was supported with 5' fully grouted rebar bolts. The fall was caused by an undetected slip.
