## Narrative

While leaning over, attaching a lifting clevis to a section of tank liner plate when a hose blew off of a main sealing water header. The stream of water struck the employee on the left side of face and shoulder, the water knocked him into a steel column behind him causing injury to left arm when it struck the column.
