Narrative

EMPLOYEE WAS ATTEMPTING TO LIFT A LIFTING JACK. HE WAS GOING TO USE THE JACK TO RAISE A CONVEYOR BELT THAT WAS LOADED WITH COAL. HE STRAINED HIS LEFT SHOULDER WHEN HE TRIED TO LIFT THE JACK.
