## Narrative

Ee was cleaning idlers along belt 4 tail pulley area when his hand was caught between the running belt and #2 & #3 idlers on the west side. He was advised in the past not to clean idlers when the belt was running. He failed to do so. He also admitted to his supervisor that he should not have been cleaning the idlers while the belt was in operation.
