WHILE WALKING TO HIS WORK SITE, THE EMPLOYEE SLIPPED ON WET, UNEVEN BOTTOM AND STRUCK HIS LOWER BACK. HE CONTINUED TO WORK AND DID NOT START MISSING WORK UNTIL 10/11/04.
