Employee stated while using a bar to scale he was struck in the mouth. He did not report to Safety Dept. requesting dental repair until 10/22/2004. I contacted MSHA subdistrict office and they called Ann at MSHA (303-231-5453) who advised to include in this narrative why it was not reported in May, 2004.
