Narrative

Injured was installing a 5' resin grouted rod in the mine roof. As he pushed it up with the bolter head it bent very quickly causing it to be fouled in the bolter head and the bolt hole. He attempted to remove it by spinning it out of the roof and was struck by the bolt.
