## Narrative

The employee was dismounting a coal hauler utilizing a hydraulic safety ladder.  Unknown to the employee, the ladder safety pins did not engage when the ladder extended.  As the employee put his full weight on the ladder it lowered suddenly and engaged the locking pins.  The jolt resulted in lower back pain to the employee.
