Narrative

When opening a dust hopper manway door under waste heat boiler 2 a lining brick fell from the hopper manway striking a ball valve which opened the valve. This caused air to be blown into the hopper which expelled hot material on to Dennis Lanceta as he was moving away. Dennis received burns to the right side of his face and his right wrist.
