## Narrative

ALLEGED to have hit a hole with a coal hauler and hit head on canopy injurying head and neck.  He continued working that shift with 5 hours overetime and the 3 additional scheduled days that week.  He did not report the injury until 11/01/04 via phone to the buisness manager.  His first scheduled workday missed was 11/03/04.  This injury is being contested to Workers Comp.
