Narrative

Employee was using diluted caustic solution to hose down mud on pad. Hose stream hit a support beam and splashed back. Some of the diluted caustic got onto employee's face and trickled into both eyes. Several days later right eye was still irritated. On 11/4/04, doctor prescribed a steriod solution.
