Narrative

After his shift of checking in freight in the warehouse the employee reported back pain. He was examined at the Colstrip Medical Center on 05/21/04. To date he has miss no time and has had no restricted work, however back pain has not been resolved. On 11/19/04 he received a spinal injuction for the pain management. He continues to perform all duties of his job.
