EMPLOYEE WAS TIGHTENING BOLTS ON PIPE AND RATCHET SLIPPED. HE FELL ON A PIECE OF PIPE, LANDING ON HIS BACK ON THE FLOOR, INJURING HIS RIGHT SHOULDER. INJURY NOT REPORTED UNTIL 11/15/04.
