Employee was dismounting a dozer when he stepped on the blade arm and slipped causing him to fall. He landed on the dozer tracks with his left elbow causing pain in his left shoulder. He did not wish to see a doctor until 11/29/04. He was diagnosed with a strained left shoulder.
