The employee was stepping off the crew bus. He tripped on a chock block and fell on the steps of the bus. He reported the incident. He sought medical evaluation and treatment on 4/22/04. He was diagnosed with inflammation and released to full duty. His shoulder did not improve and he went to a specialist and was scheduled for surgery on 12-02-04.
