Narrative

The motor operated by the injured employee had a broken coupling pin and mechanical problems preventing it from independent operation. It was chained to another motor pulling it. The coupling was being held in place by a jack bar. The pull motor stopped causing the coupling to make contact and pinch the injured's left hand between the bar and motor.
