AN 8' X 8' X 1/2"" STEEL PLATE WAS BEING USED AS FORWORK FOR A 20' X 8' TALL WALL. DURING THE CONCRETE POUR, THE WALL TIES FAILED AND THE EMPLOYEE'S LOWER RIGHT LEG WAS STRUCK BY THE STEEL PLATE AND PINNED BETWEEN THE PLATE AND SOME NEARBY CRIBBING.
