Narrative EMPLOYEE WAS PUSHING THE SKIRT RUBBER WITH THE PROBAR. PROBAR SLITHERED AND STRUCK HIS HAND BETWEEN BAR AND BELT FRAME. WET SURFACE, DARK PLACE UNDER THER SCREEN. HE DID NOT TO LOOK TO IDENTIFY THE HAZARDS FOR EACH JOB.
