## Narrative

EE was dragging water hose when they stepped on the miner cable, that had some oil and water on it, slipped and fell face down. Landed on both elbows, his hip, and knees causing them to hyper-extend in the abdomen area. They saw a Physicians Assistant who referred them to a surgeon that diagnosed them with a hernia. They hasn't lost any time (12-23-04) for hernia.
