Narrative

Employee reported on 12/15/04 he is having difficulty with his shoulders, and that it is getting progressively more painful over the years. He attributes the pain to years of repetitive motion using hammers and impact wrenches in his position as a welder/mechanic. On 01/21/05 he began Physical Therapy to help manage the pain. No time has been lost as a result of this incident.
