## Narrative

A roof fall occurred in #3 entry of the 2nd main north east at survey station 0 + 00. The fall measured 25' long by 18' wide by 9' thick. This area had been supported with 6' fully grouted and 9' double lock bolts. All approaches to this area have been timbered off.
