At the time of the incident EE was using a mechanical filter crusher. As the filter crusher was cycling back to its starting position the crushed filter became wedged between the crushing ram and the machine guard, causing the guard to raise as the ram retracted. When EE pried the filter free the guard dropped pinching his thumb and index finger
