Narrative

Employee stated that he was putting glue and bolt in the bolt hole and felt something pull in right groin area. Man was putting in 4 ft. bolt bottom-piece with glue on top of it. He was standing in sluffage off of the rib in poor body position (employee did not miss any work until 1-26-05).
