A roof fall occured in the 6 North Panel, #1 entry, starting at the inby edge of the intersection of crosscut #1 and ending at the outby edge of the intersection of crosscut #3. The fall measured approximately 90'x19'x6'. The area was supported with 6' tensioned rebar bolts. An undetected slip was determined to be the cause of the fall.
