## Narrative

Employee was washing down shop floor when he backed over an antifreeze vat and fell landing on his right hip. Employee did not wish to see a doctor until Januray 17, 2005. Doctor diagnosed lower back strain and prescribed ibuprofen and biofreeeze rub. No restrictive duty was issued. On a follow-up visit the employee was prescribed physical therapy starting on 2/8/05.
