## Narrative

Employee boarded the 1300 loader and pulled up the boarding ladder. Later in the shift his left shoulder began to ache. Seen by MD on 2/15/05 and diagnosed with tendonitis of left shoulder. Placed on restricted duty. We were notified by employee on 2/18/05.
