## Narrative

While entering the cab of d32 dragline, the ee struck his right foot against the door. this caused his left foot to slip and he fell onto his left hip. the fall bruised his left hip. 1/10/05 ee went to dr. he had a deep bruise possibly to the bone. 2/29/05 dr decided that he wants the ee to start physical therapy on 2/16/05. duties have not been restricted, still works full duty.
