## Narrative

Employee had finished loading a Pod Rockduster at cross-cut #19 in #3 entry of the 4 North Panel. She proceeded to walk past the back of the duster when a piece of rock measuring 2ft. x 3ft. x 3in. fell from the mine roof striking her in the head, left shoulder, left hand and left ankle. She sustained a fractured left ankle.
