## Narrative

Employee had placed a rug over a pipe so he could clean the area.  The pipe was 10"" dia by 28"" long and was not set into the ground.  When employee pulled rug off the pipe he pulled the pipe over, onto his foot.  The pipe struck the top of his left foot breaking it.
