## Narrative

A bundle of 6' notched wood cribbing was incorrectly loaded on the north side of the silver shaft. The proper bar was not put in place to secure the load. The crib tipped out under the bar in place as the cage was going down, and hit the crownset of the 2800 level station sending some of the crib down the shaft. MSHA on the property.
