## Narrative

A fuse box had exploded earlier in the day. The plant operator was taken to the medical center, checked for burns and released later the same day. The box was inspected, repaired and checked again. Afterwards, the quarry supervisor started the plant but the same box exploded again causing second degree burns to his face and hands.
