Narrative

EE had loaded 6 - 500' rolls of HTPE 2"" plastic pipe on hydra-crane truck. EE attempting to line up a roll on top of another. Pulled on the 1/2"" plastic strap that keeps the HTPE from unraveling. The strap broke & EE fell backwards from top of truck & landed on a wooden pallet that cut the right side of his cheek.
