Narrative

Employee was a passenger in a pickup which was driven through a hole filled with water. This incident jarred his back. He sought medical attention on 3/1/05. Later referred to chiropractor on 3/21/05. Received an adjustment & electronic stimulation to his back. His duties have NOT been restricted.
