Narrative

EE climbed a forklift boom to get up on a 2-high stack of bagged masonry products to prevent a third level stack from spilling. EE was atop the stack without fall-arrest PPE and lost footing, falling and striking head on the backstop of the forklift and subsequently back and shoulder on the forks and floor.
