Narrative

The employee was ripping hard rock with a D-11 dozer. The ripper slid off of a hard ledge or rock, and the jarring caused pain in the employees lower back, right hip and upper right leg. The employee reported on April 11, 2005 that he was being treated by a Chiropractor. The employee is working with no restrictions. Neither time nor wages have been lost.
