## Narrative

While swinging a 16# hammer against the spray tower felt a sharp pain in right shoulder. After putting hammer down - could not move lift arm without pain. Note:  Employee also had slipped on 04/12/2005 while getting into loader and noticed a slight pull in the right shoulder.  (this was recorded in log).
