Narrative

EE was installing permanent roof support on a production unit when a small piece of draw rock fell striking him on the right side of the head. He saw his Dr and was scheduled for a CT scan an April 12, 05. After Dr reviewed the results of the CT scan he was advised to remain off work until April 21, 05.
